OB/GYN Billing & Insurance InformationInsurance: We will be happy to help you receive maximum insurance benefits; however, the agreement of the insurance company to pay medical benefits is a contract between you and your insurance company. Responsibility for payment is your obligation, regardless of insurance coverage. After a surgical or obstetrical procedure, Advanced Ob/Gyn Care automatically completes a standard insurance form. All insurance companies accept this form, so you will not need to send us your own insurance form. Whenever it is possible, we will send this form directly to your insurance company. If we do not have your insurance company's address, the form will be sent to you. You should then forward it to your insurance company or to your employer. Advanced Ob/Gyn Care,LLC., will send you a statement each month until your account is paid in full. Participating Insurance Carriers: Advanced Ob/GYN Care participates with many insurance companies. Patients Without Insurance: If a patient does not have insurance or if a service is not covered by insurance, depending of the nature of the service, we will ask for payment in full prior to rendering the service or, we might ask for a deposit prior to rendering that service. Then, someone from our Billing Department will assist you in setting up a payment plan for the remaining balance due. Obstetrical Services: If you have no insurance, you will be asked to pay a $500 deposit when you come for your first visit with us. The balance of the total fees will be due in full before delivery. Our Billing Department will assist you in setting up a payment plan for the remaining balance due. Please contact our billing department to arrange a payment plan prior of your visit with our office. Credit Policies: Payment for office visits is expected at the time such services are provided. At the time of your initial visit for pregnancy, you will be given information regarding obstetrical coverage and fees. If you have any questions about policies or fees, please do not hesitate to ask. Rebilling Charges: Since the billing and mailing of statements is an expensive process, we request that non-insured charges be paid at the time of your visit. This allows us to keep the medical care costs under control and to have a more productive use of time. Patients who pay promptly should not bear the extra expense of collecting past due accounts. A rebilling charge of $2.00 per month will be added to those accounts 90 days or more past due. Miscellaneous:
