If you find it necessary to cancel an appointment, please call far
enough in advance so that we may use that time for another patient.
We recognize the value of your time, so we do our best to keep your
appointment on schedule. However, unforeseen delays may arise from surgical
procedures, deliveries or emergency patients. If your physician is delayed,
we will attempt to notify you before you come to the office or upon
your arrival. You may wait for the physician, or reschedule your appointment.
Your patience in these matters is greatly appreciated.

during regular work hours: Monday - Friday 8:30AM - 5:00PM
Please feel free to call the doctor for any of your medical needs.
We do try to answer and return your calls promptly. If you feel that
your medical problem is of an urgent nature, do emphasize this fact
to the receptionist and/or nursing staff.
Normal laboratory results may be obtained by phone from the nursing
staff; any abnormal findings will be called in to you by either the
physician or the nursing staff.
Please place all non-emergency calls during office hours, for it is
at this time that we have access to your medical records.
We have many incoming lines. You may be asked if you have an emergency.
If your call is not an emergency, you may be asked to be put on hold
when you call. Please bear with us. Your call will be taken as promptly
as possible.
When the physician is with patients, he is interrupted for emergencies
only. If you call with a medical problem or question, your doctor will
return the call at the earliest possible opportunity.
Emergencies:
Emergencies will always receive priority regardless of the day or time.
If it is a life threatening situation call 911 and /or go to the closest
hospital !!!
CALL THE PHYSICIAN IMMEDIATELY if you have abnormally
heavy bleeding, severe pain, or other urgent problems.
Prescriptions and Renewals:
All prescriptions and authorizations for renewals should be requested
during normal office hours.
Credit Policies:
Our credit policies follow. If you have any questions about policies
or fees, please do not hesitate to ask.
Fees:
Payment for office visits is encouraged at the time such services are
provided. At the time of your initial visit for pregnancy, you will
be given information regarding obstetrical coverage and fees.
Insurance:
We will be happy to help you receive maximum insurance benefits; however,
the agreement of the insurance company to pay medical benefits is a
contract between you and your insurance company. Responsibility for
payment is your obligation, regardless of insurance coverage.
After a surgical or obstetrical procedure, Advanced Ob/Gyn Care automatically
completes a standard insurance form. All insurance companies accept
this form, so you will not need to send us your own insurance form.
Whenever it is possible, we will send this form directly to your insurance
company. If we do not have your insurance company's address, the form
will be sent to you. You should then forward it to your insurance company
or to your employer. Advanced Ob/Gyn Care,LLC., will send you a statement
each month until your account is paid in full.
Participating Insurance Carriers:
Advanced Ob/GYN Care participates with many insurance companies. To
make sure we are a provider for your insurance company, please call
our office at 860-409-1930 for verification.
For some, the amounts these companies pay us will be accepted as payment
in full.
However, patients will be responsible for any services not covered
under their contract or for services rejected by insurance.
Patients are also responsible for any co-pay or deductible amounts.
If your insurance carrier requires a referral for our services or pre-certification
for any services, inpatient or outpatient, you will need to notify them
directly prior to these services.
Deposits:
If a patient does not have insurance or if a service is not covered
by insurance, depending of the nature of the service, we will ask for
payment in full prior to rendering the service or, we might ask for
a deposit prior to rendering that service. Then, someone from our Billing
Department will assist you in setting up a payment plan for the remaining
balance due.
Obstetrical Services:
If you have no insurance, you will be asked to pay a $500 deposit when
you come for your first visit with us. The balance of the total fees
will be due in full before delivery. Our Billing Department will assist
you in setting up a payment plan for the remaining balance due. Please
contact our billing department to arrange a payment plan prior of your
visit with our office.
Rebilling Charges:
Since the billing and mailing of statements is an expensive process,
we request that noninsured charges be paid at the time of your visit.
In this manner we can cooperate to keep the medical care costs under
control and to have a more productive use of time. Patients who pay
promptly should not bear the extra expense of collecting past due accounts.
A rebilling charge of $2.00 per month will be added to those account
90 days or more past due.
Miscellaneous:
1. For your convenience, we accept Credit Cards (Visa, MasterCard,
American Express and Discover Card), Debit Cards, Checks and Cash.
2. Any overpayment by you or your insurance company will be refunded
promptly.
3. Lab tests - such as blood work, Pap smears and other tests - ordered
by Advanced Ob/GYN Care will be billed by the laboratory. Any questions
you may have with these bills should be directed to the lab that performed
the test.
4. If you cannot keep a scheduled appointment, please call prior to
that time to reschedule.
If you fail to show up without notification for three consecutive appointments,
we will no longer be able to accept you as a patient.
If your account has been turned over to a collection agency, Advanced
Ob/Gyn Care will no longer be able to grant you medical services.
